Transitions Homeless Center

Transitions’ mission is to engage and equip homeless adults of the Midlands to transition into stability and permanent housing.

Transitions Homeless Center is a 260-bed facility in downtown Columbia providing emergency shelter, residential programs, and on-site services for adults experiencing homelessness. As the only low-barrier shelter in the Midlands, we welcome individuals in need of safe shelter and support, regardless of addiction, mental health challenges, or past criminal history.

Our work goes beyond providing a bed and a meal. Through coordinated case management, housing navigation, employment support, healthcare access, and daily meals, we help individuals address immediate needs while building a path toward income, stability, and permanent housing.

Since opening in 2011, Transitions has:

• Served more than 36,121 individuals

• Provided over 2.6 million meals

• Secured permanent housing for more than 4,292 people

• Supported 13,926 individuals in moving to safer, more stable living situations
